Custom wordpress registration form with a payment button

My client is an organized adult sports league and needs the ability to have people register for a league on their website. I built them a simple site using wordpress but could not find any plugins that would suffice for what we need.
An ideal form would look something similar to this :[url removed, login to view]
The attached file is the paper version of the sign up form that is being used now.
There are many different teams and seasons, so being able to have a drop down or radio button selection would be ideal.
After the form has been filled out, I would like them to be able to pay via paypal standard webpay or a better suggested system.
