Convert Sharepoint 3.0 Library to Infopath Forms Services on Hosted Sharepoint 2013

We are currently running a self-hosted version of Sharepoint Services 3.0. The current Sharepoint environment consists of multiple sites and subsites along with many lists and document libraries. With an effort to migrate services online, we have established a hosted Sharepoint 2013 environment from Microsoft on their Office 365 platform. This environment is up and running, we simply need to start migrating data. This project is NOT for a full sharepoint environment migration. Instead, we are looking for a talented individual to migrate one particular custom list (expense reports tracking application), it's views, workflows and data to sharepoint 2013. During this conversion, we want to migrate away from using custom forms on this list to an infopath form. Migrating in this direction will alleviate multiple duplicate form fields and redundant entries. I have attached several snapshots of the existing environment and where we are at this point in the conversion to 2013. Again, we are looking for a talented individual who can finish this task/project. Our current resources are spread too thin. Please reach out to me for any questions.
