Create referral status widget/plugin for wordpress site

I would like a "Referral Status" widget installed into my wordpress site. I want it do be exactly like the "drop box" referral program on the [login to view URL] (see attached for what this looks like.) once customers register on my site, go to [login to view URL] & login as username: hellodolly password: hellodolly I would like this referral program to be located in their account (admin section) but I would like to be able to choose which users have this widget in their account. I can create a form in formstack for this or you can create form & database. I would like selected users to invite there friends email address, contact name & phone number. If that email address has been registered in my database already, I would like the following message to appear " has already been invited by someone else." when the user enters in a unique email address the referral will sit in the table with an icon (see above) showing them the status of the referral. I would like an email letter to be sent to the referrer & the referee. When the refferee submits a creative brief with jstache (I use formstack - [login to view URL]) I would like the relevant symbol placed next to the referral. I have tried to explain this as best as possible. please ask questions if you don't understand.
