Application Menu Buttons

I need new menu buttons for a web application. It is a production application and includes 23 icons in a menubar. The application is a pharmaceutical dispensing application for doctors to use in their office.
They are currently laid out in a windows explorer like format, which I like, but am looking for something that makes the system look more like a desktop application rather than a web based system. Also, the images I'm using don't match. I need a complete set that look like they are meant to go together.
I'm open to ideas on how to layout the menu and have flexible options from doing straight HTML, JavaScript MouseOvers or using the current [url removed, login to view] javascript dynamic menu (preferred).
Not a fan of Java Buttons or anything animated
Buttons need to remember state (i.e. if once clicked, have a different color)
Buttons must remain in the current footprint (110px wide)
The menu buttons / icons need to represent the function
