Custom API Client Intake Form

I have a custom API Client Intake Form that I need to collect data from incoming clients. Here's an example of the form:

The form has a few different functions that need to be achieved with API:

-The form data needs to be sent to an Infusionsoft CRM via API (currently happening, needs a few changes to work properly)

-The form needs to have an upload function that syncs with Infusionsoft CRM via API (currently happening)

-The form needs to tag the contact with the correct referral partner code from a URL parameter (NOT currently happening, but needs to)

-The form needs to auto-fill some portions from URL parameters if they're in the URL line

I can give more details if you can help out. As I said, this project is one that requires knowledge of the Infusionsoft API or PHP/API development in general so you can learn as we go with this project.
