Authorized Visitor and Entry Logging database

I require a website with a database back end with at least 3 tables for use on a computer as well as a smart phone. Table 1. Users. will contain users/admin and pin numbers and their permission levels for editing the other tables Table 2. Visitors. will contain about 26 fields, including items such as first name/last name/ID numbers/phone numbers etc... Table 3. Log. will contain visitor entries with Visitor ID, Time In, Time Out, Badge ID # ect... The tables/fields will be accessed or edited on several pages. These pages will be able to search for Visitors in the access roster and return the results with conditional formatting. I will send details of these pages to interested parties. There will be Admin pages to fully edit the tables There will be User pages to search and view Visitors, enter visitors into the log and sign them out. All add/change/deletes performed by Users or Admins on the tables will be logged in a separate table Further details are available upon request but due to the sensitive nature of the details, I will not be able to put them in the bid request.
