could be because: 1 a plugin that handles cache or logging system was installed 2 is your website using SSL? there must be something to be configured 3 different password? 4 PHP related POST function handling I can simply access your SQL and just key in a new password that you can use to access the wordpress admin, just feel free to change your password again once you have the access. Simple job actually.
