I created part of a input page for a db and know what I want as far as what goes in and out, now I need some one to create it for me please read all attached files excel has sample reports and sample data needed to create sush reports the access file has the main input page where it is going to be use the must often the other pages are for maintnace this access file it is only to lay the page none of the boxes are tied to anything there is an area where you would have to choose one check box or the other one been good and the other been bad also jpg file is a screen shot of access file with comments of what should any of the input boxes should do. the db is to run in MS Access 2003 later may conver to a web base db 3 files inside the zip db, image and excel please look and read all 3 before you accept thank you ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law.
