Aditional modified submit page with api

I want to creat a aditional submit page that got more and specific data fields including google map with API. Submitpages are conected to mysql database.

On my current submit page therere has to be implemented a alternative click and unclick box to alter between current submitpage and the aditional one with more features.

The category fields has to be "locked" or default with one category only: product evaluation

All articles from this specific category has to be desplayed on indexpage as well as a subpage.

Aditionalfields will be:

Shope name(where product is bought)

Streetadress

Postal number

City

Map(with google api)

Server information: Recently upgraded to php5 and changed to safemood off

webbpage: language is swedish
