Let me guess, either you are asking to code the form to create a PDF with data submitted in the form and send it to site owner OR you are asking to take FORM and program it to work with .NET web-based system to generate PDF by sending information in background. So, I would like to ask important questions about some points. === QUESTIONS === 1 - You referenced ABCPDF but that is specific to ASP/.NET. Is it really compatible with PHP too that you were telling about it? 2 - You said existing version of "software" is on the site but the live site form page is in PHP. Now, after submission it just says thanks. What does the page has to do with PHP form with ABC software of .NET? === If you can explain me current background scenario then I can make the work for you and I can also put validations, improvements in the form to make it look more professional.
