Access app, matching facilities with criteria

This is my first experience with rent-a-coder, so please forgive my noobieness. This is a (hopefully) preliminary project using Access to assist in an eventual web-driven app compatible with mobile devices such as blackberry/iPhone. I work doing medical referrals. The facilities to which I refer offer different services to different populations. For instance some work with adolescents while others only adults, some treat X while others treat Y. When I meet with a client I match the client with the appropriate facility. What I want the application to do is match the client with the appropriate facilities for me. It should store information that describes each facility in the database. Then I want a GUI interface that allows me to pick the client attributes and the application will then match them with the appropriate facility. For instance, If the client is an adult seeking treatment for X I could use the GUI to choose "adult" and "x" and the application would give me a list of facilities that treat adults for X, and it would not show me facilities for adolescents or "y". I have a good part of the application roughed out, however I am stuck on the coding that actually does the work of? matching.? A crude GUI is in place, the database is designed, and I'm able to manually create reports at this point, but it isn't easy or "slick" so I'm the only one who can use it. I'm requesting assistance getting me past this sticking point, and design help taking suggestions on structure/aesthetics/interface. ## Deliverables I'll try attaching what I have done so far.? I'm posting this to see if it's possible to get a price I can pay for out of my own pocket since my supervisor thinks it would be useless.
