Clutter-Free Perl Form

I need assistance in processing a form page ([login to view URL]) that must utilize the Perl scripting language. My clients current form page ([login to view URL]) was a free script from Hostway web hosting and it did not allow radio buttons and/or check boxes, which the client needed to add. So I would like an action page that would basicly incorporate the new radio buttons and check boxes. Note: The client also owns the domain [login to view URL] and I think the forms contents might be mailed to that domain... not sure, yet. I just know that I had to contact the current web hosting support team to correct a problem with the free script which had a "bad referrer" error due to this issue. ## Deliverables Rent A Coder requirements notice: As originally posted, this bid request does not have complete details.
