Ajax page , login, fill form, submit, write result to db

When I do

I want someone to build a page that will allow me to enter part of that information from (HT DC 50937410129 2)

Page requires a login with password

page will have a drop down

for now only option for dropdown is HT DC and HT VA

to the right of dropdown, we have a text box for number and to right of that a textbox with amount (only 1 digit allowed)

For now I need 25 rows, but must be able to add more by editing a variable on top of the code.

I must be able to import the dropdown, number and amount from a spreadsheet (xls is prefered but i can do with csv)

on the bottom there will be a submit button, when you click submit the following happends

a db table gets populated with

login name

date and timestamp

dropdown value

number

amount

php command is sent (eg php [url removed, login to view] (dropdown) (number) (amount)

for every row you will get a soap reply

in db table we want to mark

messageID

statusID

confirmationID

ReplyAmount (AmountExcludingTax)

TaxAmount

CurrencyCode

Notes (this will be entire reply message)

on the page , we also want to display

messageID

statusID

confirmationID

ReplyAmount (AmountExcludingTax)

TaxAmount

CurrencyCode after we hit submit
