Custom E-Mail HTML & Custom Customer Feedback/Survey

Prerequisites: - This project is for a Joomla website running Joomla 3x - Custom HTML Email and Custom Customer Feedback/Survey need to be responsive (so it should look good regardless if you are viewing on computer, tablet, or smartphone) - Need to be able to provide multiple revisions, and be able to handle changes during the project. The changes may include, but not limited to o Design changes o Function changes o Layout changes Custom HTML E-Mail: I need to have a custom HTML E-Mail made to send to customers to encourage them to submit our customer survey/feedback form. Take a look at attachment to see how I want it to look. Custom Customer Feedback/Survey: I need to have a custom Customer Feedback/Survey form made for my website. When customers click on the link from the email (hence custom html e-mail requirement above), it will take them to the website to complete the feedback/survey form. Look at attachment on how I want it to look. When the customer hits submit, it sends me the form back to me via email or whatever you think might be a better method then email. Feedback/Survey Functionality: - The circles under “your overall rating of this property”, when you hover your mouse over the circles, they need to be filled in. So for example, if you select the 3rd circle; circles 1 and 2, and 3 needs to be filled in to indicate 3 circle rating. - Under “when did you travel” I need to be able to add/remove the info here. For now I need customers to be able to select January 2014 through September 2014. - The green question marks – when a customer either clicks on it, a overlaying window needs to open to provide more details. This text needs to be editable so I can change the text in the future if I need to, and it needs to be HTML friendly, so I can add hyperlinks in it if need be. - Our Honest Policy! Learn More – same requirement as the green questions marks. - I also need to be able to add additional items like paragraph text, multiple choice, checkboxes, and choose from list, etc. If you have any additional questions, please feel free to ask.
