AJAX style Survey / check list

This will be an survey with 15 questions.

Each questions will have a yes, no, not sure columns as the answers.

Each answer will be scored as the following: Yes will be +2, No will be +1 , and Not sure will be -1

As the user will clicked graded out Yes, No, No Sure graphics that will change to color once selected.

The script will keep a running count. Once the user fills out all the questions they will click a Check Answers button.

The page will then refresh with there score with the answers they selected no or no sure. Below each questioned they selected no or unsure will be a short description as to why they should have listed yes.

Those reasons can be imputed from a config files or within the same code just let me know where to enter them by leaving good comments.

This survey should be done in the spirit of web 2.0 style.

They layout should be a centered table with for columns such as:

[ Question ] [ Yes ] [ NO ] [Not Sure ]
