AJAX Form

Just paid someone to create a form for me on my site, they ripped me off.

Requirements:

- Fix form that has already been created. It is sending me an email everytime someone enters the website.

- Form should send an email to a certain address if No is selected and redirect the person to a different page

- Form should send an email to a certain address if Yes is selected and then a message should change on the form and say "Register or Login to the right"
