317747 Pop up Form

I need a form that will pop up when a visitor goes to my website that will ask them "For additional information on our services please complete the short form below and a representative will contact you with in 24 hours" it will then ask for there name email and phone number. I need this form to pop up and not be blocked by pop up blockers os i need it to be embedded into my existing php form. I need this created within 2 days, if not delivered in the specified time frame a complete refund will be issued.
