Create Forms in PHP

I need to create a new section of my site. There are several forms that need to be created. Currently the site uses a Pear db layer and Smarty template system. You can see the site here: [[url removed, login to view]][1] and if you click on Form a Corporation, you can see the forms. There are several pages. I will need to create a whole new similar section similar to this, but it will be about foreign corporations. I am attaching some documents that will help show a little about the form that will be needed. The new section should use the same general format and programming as the current site.

I need a person or company that is good with Pear and Smarty and can work on this site on an ongoing basis. Overall, I would be paying approximately $100 per week, but only for someone that is fast and good.

1. An understanding of English is needed, because often there are many small details, and we must be able to communicate efficiently.

2. It is helpful if you have at least 2 or more people on staff so that I can always communicate with you when needed.

3. You must have experience with Pear and Smarty (especially Pear). I might consider moving away from Pear, but certainly this would be a separate project that would take time.

4. Simple cut and paste bids will be ignored.

5. Your bid should be based on adding the new section to the site. Feel free to ask questions.

## Deliverables

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.

2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):

a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.

b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.

3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
