Beautiful PHP Form with custom Email recipients list

Looking for a certain someone to design a beautiful form for data entry on an Auto Transport website and landing pages. The form will include a "micro-form" that includes one field of data entry just to get people started (in an area of approximately 380px x 210px). The main form that loads next will include 3 sequential steps. 18 fields total. For functioning examples of what I am looking for, please see: [login to view URL] [login to view URL] Things I like about these forms: -status bar that updates (3 step process) -large simple data fields (feels like a mobile app, only the essentials) -the help center in the right column (that would be an added bonus) Ideally the form would be customizable at my request (small visual changes to AB test response rates). I would like it to be able to simply paste the code into an HTML box within a WYSIG editor or WordPress (a database can be installed if that is necessary). Email Group Rotation: Finally, I will need the ability to customize the email list that the form data is sent to. The leads will be distributed in small groups. An example would be the first lead is sent to email group A (6-10 emails), the second lead will be sent to group B (again 6-10 emails) third email to group C then the fourth is sent to group A again and so on. Essentially the data is sent to a rotating queue of email groups, each taking their turn. **Bonus: If you could include a separate price to integrate Make Model Year information for the form so that the Make automatically updates the Model options and Model updates Year options.
