Build an app for geo locate pictures

I need an app that does the following: - user goes out to the field to find some pipes - take geo located picture from inlet and outlet - write small description on that pipe - fill some checkboxes for shape, diameter, material After this, take the data an fill a form like attached file. Use the data collected in app to populate the form, preferably in word format. Then if online upload to server/cloud if not store the form for future upload. So summing, the app should take a couple of geo tagged pictures, add them to a form, collect some basic text info, entered by the user, into the same form. Upload/store it and move to the next.
