Create Access database from a hard document file with report printing options

My company is currently tracking data on a sheet of paper. I am wanting to start logging this information into Access. I have very limited Access knowledge; therefore, I need some help. This what I am looking for (I'll do my best explaining but I am available for a phone discussion if you would like): - a form that allows the user to enter the information found on page 1 of the attached document. - somehow the image on page 2 and page 3 of the attached document needs to be available because the numbers shown are used to fill in some areas of the report on page 1. I am not too sure the best way to do this so I open for suggestions. - once this gets created, I need the database locked down fairly well to keep employees from damaging the database. All I really need unlocked is the form to enter the information and the form or whatever is used to print the reports. - I am not sure the best way to be able to print the reports, but I am open for suggestions. One idea is have a drop down menu letting the user choose what report print based on information on page 1 of the attached document. I hope this is helpful. In a nutshell, I want to turn the attached pdf into a Access database form allowing an end user the ability to print reports from the data. I am willing to answers any questions or discuss further in depth during a phone conversation. Deadline is by January 1st. Thank you for looking at my project.
