I like this simple website

It is simple, mobile friendly and I want something similar but not the BloodWise logo on top and different images
Menu link (top left) only to what you see in front of you, not another site.
I have images for the front page, I want a gallery link.
User sign up page with capability for me to respond to all signups.
I count 5 or 6 pages total, small amount of content, nothing fancy.
No facebook, gmail or twitter links.
Need link included to download or print off the registration form.
They have gallery at foot of page, I would like it to have its own link/page.
I want the area with the 3 cars (but a new image) to be visible on every page.
Each link must be a drop down list, not as it now where it goes to a new page with link and no way of getting back home.
User can pay online (donate) and then they get a confirmation page with their details entered which they either save or print off.
The new image I provide for the top of the page will fade between 4 or 5 images, so every few seconds a new image appears.
I look forward to hearing from you, to prove you read the spec please tell me in your proposal where I want the gallery to be.
