Create a layout for ipad and ipad mini application

hello i need a GUI for an ipad normal and ipad mini application with elegant and professional graphic's design.I need single layout for both ipad mini and ipad [login to view URL] you create graphic's another coder will code an application with that graphics. This is the application detail " I'm after having some one write a app for IOS that will eliminate the use on carbon copy that we currently use . The app must have a individual log in that is password protected . The purpose is that the technician will arrive on site log on by pressing log on this will email our office with the start time and GPS coordinates .The technician will then proceed to fill in the document . Address , job number , contact phone number email . Then the description of the work done on site and what needed . If its a COD job the job sheet will turn into a receipt and be emailed to the client. The app must also calculate the cost by adding up the fields entered by the tech . And include the credit card surcharge if applicable .
