Custom USB hub PCB design

Relatively simple electronics project required: we need a PCB which is specially designed for us, operating as some USB 2.0 hubs. Exact sizes and requirements will be communicated to the winner, but the main requirements are:
* a series of seven USB 2.0 hubs on a single PCB, powered by a single molex connector;
* each hub having several USB type A sockets mounted on the PCB
* dimensions of the PCB to be discussed, having several mounting points to physically secure it
* all parts need to be readily available from common manufacturers
* ideally the PCB will use as few layers as possible
* no other requirements: no LED status lights, for example - just USB sockets!
Minimum outputs:
* Gerber file, or equivalent, so that PCBs can be manufactured by standard means
* Parts list for said Gerber file
Ideal outputs:
* a manufactured and tested PCB of the agreed design delivered
I won't be excluding people who cannot send a tested PCB of the design, but it is strongly our preference. Bids will be evaluated accordingly, to take into account the extra cost of manufacture and delivery: so please state whether or not your bid includes it.
We are also working to a relatively short time frame. Please include information on how quickly you would be able to deliver the project, and any evidence of similar previous products will definitely help.
