Data Entry for Registration Forms

We have in possession about 40,000 (maybe more) registration forms that we need to have compiled into a spreadsheet(s). We need the following columns

First name

Last name

Date of birth (if listed)

Sex

Address

City

State

Zipcode

email

Event Date (if listed)

Forms are all hand written. We prefer a hourly project with daily email updates of the new data added. Please provide an approximation of how many forms you believe you can average per hour based on the above fields.
