In the form 801A - there are three sections. Section 1 - Notice of intent - 1. Municipal code - in pg 6/7 - there are around 850 municipal codes. Trust you have excel sheet with that data so we can create a municipal master table and import the data and refer that in this field 2. Municipal name - based on code - we will fetch the name 3. Applicant Name - will be free text 4. Tax year - will be free text Section 2 1. Business address - will be free text 2. fields - a,b,c,d,e,f will be in a sub-form and you can enter as many rows as you want 3. Totals will be READ ONLY computed field Section 3 1. Property Tax Rate will be free text 2. assessed tax $ will be free text 3. tax year will be referred to the field tax year in section 1 4. signature/date will be blank 5.
