ASP HTML Website Form w/ Checkboxes and Drop Downs

This project should be cake. All I need is two forms that can be worked into a basic html page which will be hosted on a Windows server using ASP technology. The forms needs to be ones which will directly send the form submissions to an email address with a nice clean layout oranizing the submitted data. There will be both checkboxes and drop-down menus. Drop down menus should be developed in a way that is easy to edit current drop down options, add new ones, and change the order of the drop downs within simple code. The code should include a required fields option so its easy to edit the code to define which form fields are required (you only need to code out a couple of these for each form and we'll take it from there). If the user trys to submit the form with missing required fields it will prompt them with either a pop-up dialog box or right ont he page...it doesn't have to be specific about whcih field they missed, but if prompting the specific required missing fields is easy enough, please bid out that additional cost. The forms should run of a form code that can be embedded in an html page and an ASP file which runs the form...they should not reference any other external files or run the e-mail addresses through anythign else, as these emails and data submitetd must be kept private and unrelated to the developers after they finisht he project. Your deliverables should include one html file with the form code one asp file that runs the form, and do not rely on anythign else, except for being on a windows server. Easy editing of the form code and ASP file with basic code knowledge is paramount, as these forms will be tweaked after you finish them. I am attaching a PDF which shows the two forms I need developed. The blue text on Form #1 is the sample drop downs I'd like you to work up, but I must eaisly be able to edit them in the code later on. Thanks!
