I am needing a children's memory book designed similar to a baby book where children can write in their answers to questions in the book, draw pictures in designated spaces, and add photos or memorabilia. I have uploaded a few pictures of the type of book I am looking for. The book can be 8x8 or 8x12.

Please use the following information for your submission. The winner of the contest will be hired for the book design.

Please design 2 pages
Page 1 information should include spaces for:
Name, age, what I want to be when I grow up, space for child to draw a picture of their family

Page 2 can include whatever you would like. I am really just wanting to see how you would lay out a memory book to see if we could work well together. You could design another page of the book or a book cover

Winner of the contest will be hired to create the entire book. (it will be roughly pages)
I will also be creating a second version of the book that will be similar once the child's book is ready.
