I need a report template (Size: US Letter)
It must contain two pages:

First:
A status page with the following fields:
-Report title
-Conclusions
-Issuer (Name and email)
-Addressee (Name and email)
-Distribution list (list of email address)
-Status (% of observations ok and pending)

Second:
A grid with 6 cells (observations), each one with space for a picture and the following fields:
Location, Description,Obs. No.,State

Each page must have a header with the company logo, project name, report number, date, address, client, issuer, and receiver.
Also a footer with the company address is needed in each page.

Please focus on proportions, font type, size of each field, etc.

Optional: Report can be made using MS Word but if you implement it using the pdfmake.org format (json) you get a 20 USD bonus.
